This is a complex form I created for a hospital. I have experience in creating forms with lots of information and details needed. These were ordered as continuous pin-fed carbonless forms after completion. They still contact me for updates and revisions as needed.

Designed this brochure for a plastic surgeon, using only two PMS colors to keep their printing cost down. Getting the right photos to be conservative but still show skin was challenging. The brown and blue gives it a calming trusting feeling.

Designed this funeral program and it was greatly appreciated. I was given the photos and wording and came up the layout and background images. The gray background is brushed steel and they just loved the effect it gave the program. We printed them on 100# glossy text so they made nice keepsakes.
