I'll be honest: most of my experience comes in the way of illustrator, but with that having been said I often have a lot to say about the way I think things "should" look. But my "shoulds" have a lot to do with things like balance, flow, visual use of space, geometric placement, etc. Ultimately, it's up to the client to make the final call on that, though. Currently, I'm working on a children's book where the author allows me a lot of input. Frankly, I think that greatly increases any author's chances of making the book a big hit... and that's exactly what I (and other artists) need, because I will make my money not on this one, but the next one. If the client allows me to, I have a good chance of selling their book from "across the room." If they do their part, and write a good story to go with it, we all win. Surprisingly, all of these components aren't always aligned.
