Medical Foster Home (MFH) Program Coordinator
Overall function of coordinator includes program development, recruitment, and development of the foster homes, recruitment and placement of veteran residents, case management, development of policies and care processes, and coordination with Home Based Primary Care & Mental Health Intensive Case Management interdisciplinary treatment teams.

Recruits appropriate caregivers and residents, establishes intake processes, establishes oversight responsibilities, and maintains a record-keeping system for the program.

Plans & implements bi-annual education conference for MFH caregivers, as required by MFH handbook

Markets the model of care and reaches out to internal stakeholders and community groups/agencies to promote the MFH.

Provides training to caregivers, students, residents, interns and fellows from other disciplines regarding the benefits of a medical foster home.

Serves as a mentor to other VA MFH program coordinators and social workers

Navigates VBA and VHA to expedite pension claims, allowing Veterans the ability to afford the MFH program.

Reviews all data, subjective and objective, and makes a clinical assessment, identifying needs and strengths and efficiently responds to MFH monthly & quarterly reports.

Developed program to 30 placed Veterans in 13 VA inspected MFHs. Veterans include OEF/OIF, Women, and Priority 1a, and new unique to the BPVAHCS; establishing a cost savings/diversion for Veterans that would otherwise be placed in a Community Nursing Home under VA pay.
